Program:  Homeless Outreach FSP Team (HOT)  

Position Summary

The Registered Nurse is a member of the Homeless Outreach Team’s multidisciplinary team and is responsible for; assessing physical health needs; promoting health literacy; making appropriate referrals to community treatment providers; providing management and administration of medication; providing a range of treatment, rehabilitation, and support services; and sharing shift-management responsibility with other staff using a trauma informed approach in all duties. The Registered Nurse will support homeless outreach clients in the community to bridge to needed services, as well as Homeless Outreach Full Service Partnership clients on an ongoing basis. 

Hours/ Salary

Part-Time, 25 hours per week. Anticipated hiring range is $43.00-$56.00 per hour. Includes retirement (with up to a 5% employer match) paid sick time and a variety of helpful employee perks as well.   (*Full Salary Schedule $43.00-$56.00 per hour)

Position Summary:

The Registered Nurse will support homeless outreach clients in the community to bridge to needed services, as well as Homeless Outreach Full Service Partnership clients on an ongoing basis. 

Target Population:

This position works directly with adult clients who are homeless or at risk of homelessness and have a severe and persistent mental illness. Clients may also have co-occurring disorders of mental illness and substance addiction issues, and will have moderate to severe medical issues as well. 

Responsibilities and Duties:

1. The  Registered Nurse conducts the client-centered comprehensive nursing  assessment, including assessments of physical health, cognitive  functioning, safety, use of substances, education and employment, social  development and functioning, and activities of daily living (e.g., self-care, living situation, nutrition, money management). The Registered  Nurse creates a treatment plan to address areas of concern and supports  the client with following up on those needs. 
2. The Registered  Nurse promotes health literacy by providing education on medical  conditions and pharmacological and non-pharmacological treatment options,  within their scope of practice. 
3. Under the  direction of the Medical Team Lead and in collaboration with the Homeless  Outreach Team Program Manager; implement psychopharmacologic and medical  treatment and medication policies and procedures including transcribing,      administering, evaluating, and recording prescribed medications; evaluate  and chart psychotropic medication effectiveness, complications, and side  effects, and arrange for required lab work according to protocol.
