Job Description
- Provide leadership, direction, and supervision to all direct reports. Coordinate schedules to ensure service hours are being met for all individuals served.
- Assist in obtaining and arranging for additional services such as medical, dental, psychiatric, and any other services necessary. Attend service appointments as needed.
- Act as a liaison to and communicate on a regular basis with the individual served, family members and/or guardians and other service providers.
- Facilitate the development ISP's.
- Ensure the safety and wellbeing of the person’s served. Continually monitor and respond to health & medical needs, nutritional needs and promote a healthy lifestyle, etc.
- Provide transportation for the persons served as needed
- Provide on-call assistance and response to emergency situations

Requirements
- First Aid and CPR Certification required before starting employment - we will help you obtain this!
- MAP (Medication Administration Program) certification encouraged - we will help you obtain this!
- Bachelor's Degree preferred - Associates Degree or High School Diploma may be substituted for relevant work experience
- Valid driver’s license and access to a vehicle required.
- 2 years of relevant experience
- Personal commitment to service quality and excellence
- Proven leadership skills and supervisory experience
- Excellent verbal and written communication skills and demonstrated ability to work as a part of a team.
- Knowledge of services and supports for people with developmental disabilities is preferred
- Ability to lift up to 30lbs
- Ability to read and write in English
- Familiarity with computers
