Job Description

• Provide leadership, direction, and supervision to all direct reports. Coordinate schedules to ensure service hours are being met for all individuals served.
• Assist in obtaining and arranging for additional services such as medical, dental, psychiatric, and any other services necessary. Attend service appointments as needed.
• Act as a liaison to and communicate on a regular basis with the individual served, family members and/or guardians and other service providers.
• Facilitate the development ISP's.
• Ensure the safety and wellbeing of the person’s served. Continually monitor and respond to health & medical needs, nutritional needs and promote a healthy lifestyle, etc.
• Provide transportation for the persons served as needed
• Provide on-call assistance and response to emergency situations

Requirements

• First Aid and CPR Certification required before starting employment - we will help you obtain this!
• MAP (Medication Administration Program) certification encouraged - we will help you obtain this!
• Bachelor's Degree preferred - Associates Degree or High School Diploma may be substituted for relevant work experience
• Valid driver’s license and access to a vehicle required.
• 2 years of relevant experience
• Personal commitment to service quality and excellence
• Proven leadership skills and supervisory experience
• Excellent verbal and written communication skills and demonstrated ability to work as a part of a team.
• Knowledge of services and supports for people with developmental disabilities is preferred
• Ability to lift up to 30lbs
• Ability to read and write in English
• Familiarity with computers
