Summary of Duties and Responsibilities:

The Community Health Worker will be responsible for assisting patients with a variety of care coordination activities. Activities will vary based on patient need. Basic functions will include helping patients to navigate the healthcare system, understand and be connected to Variety Care as a patient-centered medical home, and connecting to community resources to help eliminate barriers to care.

 

Primary Duties and Responsibilities

  1. Applies self proactively and conscientiously to Community Health Worker training program(s) as they become available.
  2. Creates a positive experience and relationship with the patients assigned to their panel.
  3. Works directly with the patient in the community, meeting at agreed upon locations or at patients’ homes as situation and safety allow.
  4. Keeps the patient(s) out of the hospital by supporting regular visits to their primary care medical provider, including assisting patients in learning how to use telehealth.
  5. Keeps the patient(s) actively engaged with their primary care medical provider.
  6. Supports the patient(s) to ensure pick-up of their prescriptions within set protocols.
  7. Engages the patient(s) to manage their own care.
  8. Supports transitions of care for all patients on the panel.
  9. Helps to keep the patients compliant with their care plans.
  10. Maintains knowledge of and continually learns more about the community cultures and values of the patients on their panel.
  11. Maintains knowledge of and continually learns about available resources according to insurance coverage or uninsured status of patients and how to help patients access those resources.
  12. Acts as a patient advocate and liaison between the patient and the clinical staff
