Job Summary:
This leader works across clinical, operational and departmental functions to orchestrate new project/program design, and plan for sustainment of services that address racial disparities and clinical care gaps, and that meet social needs affecting health. This position is responsible for building and sustaining the technical infrastructure for comprehensive, patient-centered, and efficient screening for social needs, strengthening referral processes and communications with community-based organizations to meet individual’s needs. This includes training, education, and demonstration of the Find help platform, Native + and others. Provides oversight in collaboration with Social Health Lead’s oversees the analysis and application of social and population analytics to improve outcomes.
Primary Responsibilities:
- Drives system-wide increases in screening for social risk factors, referrals to community-based organizations and closed loop follow-up, including knowledge of Salesforce.
- Leads/coordinates the development and sustainment of system infrastructure (roles, workflows, EHR design, and vendor relationships, etc.) that lead to improvement in patient outcomes in target populations.
- Coordinates system initiatives/projects that address health disparities and clinical gaps in target groups. Brings together broad groups of stakeholders and experts to assess opportunities, identify solutions, and execute improvement plans. Accountable for tracking performance to process and outcome targets.
- Uses population data, clinical and social needs databases to prioritize opportunities to improve health equity. Analyzes and reports program outcomes. Utilize Race, Ethnicity, and Language (REaL) to identify gaps and track disparities reduction in identified high-priority patient populations.
- Collaborates with patient experience, community relations and patients representing the diversity of populations served by AAH to identify opportunities to provide equitable, compassionate, and culturally sensitive care in targeted groups. Incorporates best practices in health literacy, patient education and engagement to improve clinical outcomes.
- Uses data-based decision making, standard improvement and change management processes to achieve and sustain outcomes and close health equity gaps.
Education and Experience Required:
Level of Education: Master’s degree required.
Field of Study (if applicable): Healthcare, IT, public health, business, project management, or related field of study
Years of Experience: Seven years
Describe Type Experience: : Project development, management, implementation, and evaluation. Background in healthcare operations, network development, IT, public health, social work of clinical experience preferred.
