Description

Provides management and supervision of case management and peer support staff as part of a community based multidisciplinary team.
Demonstrates effective coordination of care with internal and external providers.
Provides direct case management services in the community to parents with Child Welfare cases as part of the FITT team.
Promotes involvement in recovery oriented services and supports.
Provides immediate access to substance use and mental health services for parents.
Responsible for assessment and discharge planning for persons receiving services through the FITT team.
Develops timely, effective, individualized, service plans based upon the assessment results
Conduct and document service implementation
Develop objectives, which are measurable, stated in behavioral terms and service oriented
Maintain documentation, which is
- Comprehensive
- Complete
- Timely, and
- Consistent with standards
Maintain documentation, consistent with internal and external audit standards
Document progress notes reflective of Data/Assessment/Plan (DAP) method
Demonstrates effective coordination of care with internal and external providers
Perform home visits with patient.
Links patient to appropriate community service agencies.
Demonstrate understanding of target population’s clinical and age-specific issues
Conduct accurate, strength-based assessments that are consistent with the patient’s history, presenting problems and needs
Completes up-to-date documentation in accordance with the internal and external audit standards.
Effective Transition/Discharge Planning which is:
- Appropriate
- Timely
- Reflecting patient participation and
- Incorporates patient strengths and natural supports
Communicate effectively with other community service organizations and state/federal agencies
Provide routine, effective supervision and feedback to staff that is consistent with agency policy and staff need
Ensure that staff attends required training
Use Corrective action and performance evaluation as a tool
Review monthly financial information and assist Director in managing revenue and expenses over the course of a fiscal year
Meet or exceed productivity and timeliness standards for department
Seek out opportunities to create efficiencies through a variety of opportunities including process efficiencies, documentation efficiencies, cross training, etc
Work with staff individually and as a group (training) to improve timeliness and quality of documentation
Demonstrate ability to schedule time and prioritize multiple projects to meet deadlines
Position requires 50% or more time in travel.
