Social Worker | Part-Time | Elite Home Health & Hospice - Moscow area

What You’ll Do:

• Assess patient and family emotional, social, financial, and environmental needs (coping, safety, caregiver strain, resources, advance care planning readiness).
• Care coordination & resource navigation - connecting patients/families with community resources and coordinating with the interdisciplinary team.
• Participate in care plan development and case conferences.
• Serve as liaison between patients or families/caregivers and community agencies.

What You Bring:

• Current state licensure, or eligibility for licensure as required
• Strong skills in psychosocial assessment, counseling, and care coordination. 
• Ability to support patients and families through serious illness, end-of-life care, and grief with compassion and professionalism
• Excellent communication, documentation, and time management skills.

Wage: $25 - $30 per hour
