Primary Function:
Assist a designated participant of UESP to live at home safely by providing home management, personal care, exercise, social support and escort services, as indicated by the participant and the identified needs in the participants individual care plan.
Duties & Responsibilities:
Under the direction of the participant and PACE Home Care Liaison PCW duties may include but are not limited to:
- Activities of Daily Living (ADL): bathing, grooming, dressing, undressing, feeding, toileting, walking, transferring in and out of bed, chair, or tub.
- Assist with medications.
- Exercise Therapy including walking and range of motion; and the use and care of equipment such as Hoyer lift, wheelchair, commode, tub seat.
- Home Management: cooking, shopping, laundry, housekeeping, money management.
- Social Support: escort to appointments, recreation activities.
- Complete required UCHC paperwork.
- Communicate with supervisor and staff.
- Attends in-services as determined by supervisor.
Qualifications
Minimum Basic Knowledge:
- Able to follow care plan.
- Relate well to participant.
- Able to communicate in participants language.
Experience & Qualifications:
- Experience working with elders in their homes.
- Demonstrates ability to provide services identified in care plan.
Independent Action:
- Provide care at home.
- Report observation to clinical supervisor.
Supervisory Responsibility:
None.
