About the role
Pharmacy medication reconciliation technician is a certified pharmacy technician who supports pharmacy services by obtaining medication and allergy histories for patients that will be admitted to the hospital. Information is obtained by interviewing the patient, patient family, calling pharmacies, long term care facilities and/or other providers. The information obtained is communicated to pharmacists and health care providers to determine treatment plans and medication orders during hospitalization and post discharge from the hospital.
- Registration with Kentucky Board of Pharmacy (KY hospitals) within 30 days of hire date if you are a new registrant. If previously registered, you must have registration upon hire).
- Nationally certified as a pharmacy technician by PTCB required
- Medication history certificate by PTCB preferred. Certified Compounded Sterile Preparation Technician (CSPT) or Controlled Substances Diversion Prevention Certificate by PTCB preferred.
- 6 months of pharmacy experience
