Christiana Care HomeHealth is hiring a full time Physician Coordinator to join our dynamic and cohesive team.  The Coordinator will work Monday through Friday 8am to 4:30pm and will work out of our Smyrna, DE office completing the following:

• Retrieve physician orders
• Leverage critical thinking and problem solving skills
• Build relationships with physician offices, staff and colleagues within the Physician Coordinator team
• Identify trends in delinquent physician orders
• Reporting – generate reports out of netsmart and home care software

Requirements:

• High School Diploma or GED
• 1 year of Medical Records experience
• 2 years of computer experience
• HomeHealth EMR experience preferrable; Matrix Care, Epic, Netsmart, or Access Care.
• Knowledge of home health regulations is strongly preferred.
