Summary:

Collaborates with state and local health department staff, healthcare providers, and community partners to strengthen diabetes prevention and management efforts across the service area. Serves as a key resource for individuals with diabetes, those at risk for diabetes, and healthcare professionals by providing education, tools, and guidance that support improved health outcomes. Plans and implements community outreach and public awareness initiatives that promote diabetes prevention and control. Activities may include coordinating health fairs, community events, media outreach, and presentations to civic groups while distributing educational materials aligned with the Kentucky State Diabetes Program. Facilitates evidence-based education programs that encourage healthy lifestyle changes among individuals and families affected by diabetes or prediabetes. Programs may include diabetes education classes, weight management and nutrition initiatives, cooking demonstrations, support groups, and other community education events. Coordinates Diabetes Prevention Program (DPP) activities to ensure compliance with National Diabetes Prevention Program (NDPP) standards and assists with preparation of required program reports. Supports Diabetes Self-Management Education and Support (DSMES) services by assisting with participant plans of care, chart audits, and other quality assurance activities. Works collaboratively with program staff and community partners to support coalition efforts, strengthen community partnerships, and identify innovative strategies that advance diabetes prevention and chronic disease management throughout the district. Assists with data collection, program evaluation, and quality improvement initiatives to strengthen program effectiveness. Supports grant-funded activities and contributes to program development efforts as needed. Participates in staff training, professional development, and networking opportunities related to diabetes prevention and public health practice, and may assist with orientation and mentoring of new program staff.

*NEW* Starting Salary Range: $28.00 - $34.13 per hour

CHARACTERISTICS OF THE CLASS

CHARACTERISTICS OF A CLASS: ARE GENERAL STATEMENTS INDICATING THE LEVEL OF RESPONSIBILITY AND DISCRETION OF POSITIONS IN THAT JOB CLASSIFICATION. 

  • Under limited supervision provides work of considerable difficulty in this fully prepared level, requiring the independent application of experienced judgment and skills. Serves as a nursing resource person in all service or specific programmatic areas, client eligibility requirements, service protocols, standards, scope of practice and documentation requirements. Conducts communicable and preventative disease case finding investigations for the purpose of preventing and controlling diseases and disabling conditions and the promotion and maintenance of health populations. Performs some delegated management duties, as well as, consulting responsibilities

MINIMUM REQUIREMENTS: MINIMUM REQUIREMENTS ARE COMPREHENSIVE STATEMENTS OF THE MINIMUM BACKGROUND AS TO EDUCATION, EXPERIENCE, AND OTHER QUALIFICATIONS WHICH WILL BE REQUIRED IN ALL CASES AS EVIDENCE OF AN APPOINTEE'S ABILITY TO PERFORM THE WORK PROPERLY.

EXPERIENCE:

  • Two (2) years of Registered Nurse (RN) experience.

  • Must have RN license in Kentucky or compact state.

POST EMPLOYMENT REQUIREMENTS: EMPLOYEES IN THIS CLASS WILL BE REQUIRED TO OBTAIN CREDENTIALS OUTLINED BELOW WITHIN A REASONABLE TIME, OR IF EMPLOYEES POSSESS SUCH CREDENTIALS AT THE TIME OF HIRE THEY WILL BE REQUIRED TO MAINTAIN THOSE CREDENTIALS SO LONG AS THEY ARE EMPLOYED IN THIS CAPACITY.

  • Must maintain current RN licensure in Kentucky or compact state. Must complete required annual updates
