What You’ll Do as a Hospice Nurse Practitioner

In this role, you’ll provide expert hospice and palliative care to patients and families while ensuring comfort, dignity, and peace during life’s most meaningful journey.

You will:

  • Perform routine and emergency face-to-face recertification visits for patients entering their 3rd benefit period or beyond, including admissions, transfers, and extended stays.

  • Assess, document, and communicate clinical findings with the Medical Director, Attending Physician, and interdisciplinary team (IDG).

  • Provide physical and emotional support to patients and families, especially during crisis situations.

  • Triage consults and prioritize care based on patient acuity.

  • Deliver bedside palliative care and education for patients and caregivers.

  • Prevent unnecessary hospitalizations by meeting patient needs at home.

  • Maintain accurate and compliant documentation, billing, and coding per CMS guidelines.

  • Support quality care through staff training, continuing education, and clinical guideline development.

  • Uphold Elara Caring’s commitment to integrity, compassion, and excellence in all interactions.

What You Bring to the Team

  • Master’s Degree in Nursing and national certification as a Nurse Practitioner

  • Active state NP license and ability to maintain licensure

  • 1+ year of clinical nursing experience (hospice or palliative preferred)

  • Experience in Family Medicine, Geriatrics, or Psychiatry is a plus

  • DEA certification (if applicable for prescribing in your state)

  • Valid driver’s license, insurance, and reliable transportation

  • Ability to travel up to 50%, and lift/move up to 50–100 lbs as needed
