I. Major Responsibilities:

1.  Taking vitals (blood-pressure, pulse, temperature, and respiration) signs as ordered by physician
2.  Conducts medical monitoring activities consistent with qualifications under direction of the R.N.
3.  Measuring statistics. Recording intake and output, height, and weight.
4.  Obtaining routine lab samples that do not require laboratory personnel to complete.
5.  Helping Persons with ADL’s (activities of daily living) for which they need assistance, such as bathing, assistance with ADL’s when incontinent, grooming, combing hair, brushing teeth, shaving, putting on stockings and/or slippers, dressing, ambulation, toileting and other personal care activities as necessary.
6.  Helping individuals with equipment for their treatment such as anti-embolism stockings, bladder scanner, intermittent pneumatic compression (IPC) devices, etc
7.  Helping Persons to move, transfer from bed to chair, or transporting them to other facilities for testing or treatment.
8.  Know and follow proper protocol in avoiding the spread of germs and spores which transport disease.
9.  Observing the persons and surroundings assists the Nurse/Program Coordinator/Assistant Coordinator to assess the progress of the persons as well as any needs the persons might have for medical intervention.
10. Observe and remove any potential hazards which could cause accidents or injury to Persons.
11. Communication among the healthcare team, of which the CNA is a vital part, is critical.
12. Timely and precise communication is a key element in avoiding errors in persons treatment.
13. Trained in documentation, in observation, and in reporting, both to the Nurse/Program Coordinator/Assistant Coordinator as well as to one’s replacement at shift change, or one’s end-of-shift report.
14. Work with Persons to maintain the residence on a daily basis including chores, grocery shopping, cooking, bedroom upkeep, and other related housekeeping duties.
15. Participation in IAP development, daily documentation, Incident Reporting, Medication Documentation of medication dispensing, and other documentation as required.
16. May include transporting Persons in personal car or program vehicle, physical assistance, close supervision, and other such related duties.
17. Provide direct service and routine nursing aide care in a clinically and/or medically complex program model, designed to meet the needs of Persons who have both medical and psychiatric needs as well as to ensure a safe and clean environment.
18. Use rehabilitation values, principles and techniques, including skills training to help program Persons overcome barriers to living independently.
19. Help program meet high quality standards and provide value to Persons and community health care system.
20. All applicants must be certified as a MAP, CPR, and First Aid within the first five months of employment
