Key Responsibilities:

• Conduct comprehensive patient assessments and admissions.
• Provide high-quality, compassionate care during regular visits.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Offer support and education to patients and their families.
• Participate in an on-call rotation for one week, with an extra stipend for your dedication.

Requirements

Valid RN license in Texas.

• Minimum of 1 year of nursing experience, with hospice or palliative care preferred.
• Strong clinical skills and attention to detail.
• Excellent communication and interpersonal abilities.
• Compassionate, patient-centered approach to care.
• Knowledge of Homecare Homebase software would be an asset.
