Position Overview: Senior PCA

The Patient Care Advocate/Sales Representative plays a critical role in increasing access to home health services by educating healthcare providers, patients, and families about the benefits of home health care. This individual will be responsible for building and maintaining strong referral relationships, driving patient referrals, and representing the organization in the community. The ideal candidate combines compassionate communication skills with a results-driven mindset to help ensure individuals receive timely and high-quality care.

Key Responsibilities:

1. Business Development:

· Develop and execute strategic sales plans to identify and engage referral sources, including physicians, hospitals, nursing homes, assisted living facilities, and community organizations.

· Build and maintain relationships with key healthcare providers, discharge planners, and social workers, to drive patient referrals.

· Meet or exceed sales goals and targets for patient admissions and referral growth.

2. Education and Outreach:

· Educate referral sources, patients, and families on the benefits of home health care and the services provided.

· Provide in-service presentations, lunch-and-learns, and community seminars to increase awareness of home health care.

· Stay informed about industry trends, regulatory changes, and competitive services to position the organization effectively.<
