Community Health Worker

The role of the Community Health Worker (CHW) is to create connections between diverse, underserved, and vulnerable populations to health and social service systems. and the health care system.  Building trust and promoting encouragement are two of their key objectives. Their job allows them to help the patient access community services specifically to their needs. A Community Health worker also supports medical providers and the management team by creating community outreach programs and activities. These outreach programs are designed to promote, maintain, and improve the health of the patients and their family they serve.

Essential Functions

• Serve as a liaison between multiple service providers
• Assist with enrollment in services and community resources
• Complete applications for resources, paperwork for provider visits etc
• Follow up on referrals for pharmacy, DME, home care etc
• Assist with finding and scheduling transportation
• Deliver culturally competent care
• Advocate for members/patients
• Support clinical staff
• Administer health screenings
• Must be willing to meet patients in various settings such as home, provider practices, clinics or dialysis centers

Minimum Qualifications

• Certified CHW or certified nurse aid or licensed medical assistant
• 1 year experience in a CHW type role
• Reliable transportation
• Active Basic Life Support or CPR Certification
• Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms

Preferred Qualifications

• Experience working in a multi-cultural setting
• Experience working for a managed care plan
• Experience with kidney patient’s
• Experience with translation lines and services
• Extensive knowledge about community and available resources
• Basic computer skills
• Good communication skills
• Good organizational skills

Hourly Range: $23.08-$27.16

 

Strive Health offers competitive compensation and benefits.  An annual performance bonus, determined by company and individual performance, is available for many roles and aligned to Strive Health guidelines.
