Licensed Practical Nurse (LPN) Personal Care: Part-time Evenings

Foxdale Village, a life plan community guided by  Quaker values, is non-profit Continuing Care Retirement Community (CCRC)  located in State College, PA. Foxdale Village was founded on the values  of treating every person with dignity and respect and actively strives  to create an engaging and welcoming environment for all. We are seeking a  part-time Licensed Practical Nurse to join our caring and dedicated personal care team (Darlington House). This  LPN position generally works 3, 8-hour evening shifts per week, and every other weekend, and also participates in the holiday work rotation.   

Licensed Practical Nurse (LPN) Job Duties Include:

• Develop, review, and update individualized interdisciplinary care plans.
• Administer medications as prescribed by the physician following policies and procedures.
• Safely use specialized clinical equipment and implement nursing care procedures.
• Effectively communicate to assure continuity of care and efficiency in work responsibilities.
• Supervise, mentor, train and orient certified nursing assistants. Instruct as needed in order to ensure safe and competent care.
• Accurately document using PointClickCare electronic health records system.
• Participate  in the development, review and update of an individualized  interdisciplinary care utilizing the primary caregiver’s approach.
• Foster  a collaborative approach in working relationships with other Health  Services staff, as well as the residents, their families and friends,  and other departments.

Requirements

Licensed Practical Nurse (LPN) Minimum Qualifications Include:

• Graduate of an accredited school of practical nursing.
• Licensed to practice nursing in the state of Pennsylvania as a Practical Nurse (LPN).
• The possessor or have the ability to obtain a valid CPR card.
