About the role
DH GI is looking for an APP to join our team. Monday-Friday 8am-4pm or option to work 4, 10 hour days.
The APP will function as a dedicated hepatology-focused provider, working collaboratively with physicians, nursing, care coordination, and ancillary services. This role will emphasize proactive, longitudinal management rather than reactive, episodic care.
Key responsibilities will include:
- Post-discharge follow-up for hepatology patients
- Ongoing management of chronic liver disease and cirrhosis
- Medication reconciliation and adherence monitoring
- Patient and caregiver education on disease management and warning signs
- Coordination of labs, imaging, and specialty referrals
- Timely outpatient intervention to prevent decompensation
- Monitoring compliance with appointments, medications, and treatment plans
