REGISTERED NURSE (RN) HOME CARE ON CALL WEEKENDS
Summary
Provides clinical support and assists in the coordination and delivery of patient care services during or after normal business hours. Performs phone triage, direct patient care activities, data/order entry, clinical record reviews, and other clinical tasks related to the provision of patient care.
Job Duties
- Performs telephone triage for patients calling during or after normal business hours utilizing critical thinking skills and sound clinical judgement.
- Performs assessment of patient status and addresses immediate needs related to physical, psychosocial, and environmental factors.
- Performs data/order entry, clinical record reviews and other clinical tasks related to the provision of patient care
- Provides hand-off communication to RN covering next shift, field staff, and/or Patient Care Manager as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or phone call.
Minimum Qualifications
- Specialized Diploma NLN approved/accredited nursing program or
- Associate’s Degree NLN approved/accredited nursing program
- 1 year direct care nursing experience or
- 1 year home health and/or hospice experience
- Demonstrates knowledge and ability to care for patients based on the population served
- Demonstrates good critical thinking skills and sound clinical judgement
- Demonstrates analytical and problem-solving skills
- Ability to perform duties with accuracy and attention to detail.
- Demonstrates excellent written and verbal communication skills.
- Demonstrates excellent customer service skills
- Demonstrates team behaviors and ability to work independently.
- Demonstrates ability to multitask and work with multiple interruptions and adapt to changing priorities.
- Demonstrates evidence of competency as per department/unit specific competency assessment criteria
- Demonstrates excellent time management and organization, and prioritization skills to manage workload.
- Demonstrates proficiency in basic computer skills
- Knowledge of agency and network clinical policies, procedures, and resource materials
- American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
- DL - Driver's License_PA - State of Pennsylvania Upon Hire
- RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire
Preferred Qualifications
- Bachelor’s Degree Nursing
- 1 year home health/hospice experience and
- 1 year nursing experience in a clinical office setting or phone triage/on call responsibilities
- AMB-BC- Ambulatory Care Nursing ANCC - State of Pennsylvania within 3 Years
Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
