Job Description

The Medical Director of Home Based Primary Care will be expected to participate in the development of programs that serve our patients in home and home-like settings. The Medical Director of Home Based Primary Care is responsible for developing policies, procedures and work processes to ensure the delivery of high quality, in home care, in accordance with best practice and standard of care. They will build, support and mentor the community-based care teams. The Medical Director of Home Based Primary Care will participate in and guide strategic planning for community-based services. Goals are to optimize the care of patients in the community by measuring and tracking outcomes against goals, optimize working relationships with community partners and other KVH departments.

Qualifications

• Completion of appropriate License or Certification, including:
  • Washington State License
  • Drug Enforcement Administration
  • National Provider Identifier
• Completion of residency
• Board Certification in Family Medicine, Internal Medicine, Geriatrics or Palliative Care.

Wage: Wage is $141,236, this is a guaranteed base salary. Plus 10% employer contribution to pension plan. 

FTE:  0.5= 20 hours/week
(Position has the opportunity to increase to 1.0 FTE with additional responsibility in a primary care clinic.)
