Current, unrestricted RN license in state of residence and ideally compact states.
Wages: $68,000-$78,000 annually; geographical location impacts pay.
Monday - Friday
8:00 am to 5:00 pm (subject to needs)
Six paid holidays
Must pass post-offer, pre-employment drug background tests as allowed by state, federal, local ordinance, statutes and licensing/accreditation requirements.

Job Title: Patient Care Nurse

General Functions
Within the defined scope of nursing practice, assist pharmacists and staff members in reviewing and providing pharmaceutical care services to the patients of Amber Pharmacy and Hy-Vee Pharmacy Solutions. This includes but is not limited to oversight, monitoring, and proper documentation of patient medication and disease therapy management, as well as acting as a resource for subcontracted nursing services.

Primary Responsibilities:
Coordinates and aligns patient services with field resources as well as third-party vendors.
Evaluates the entire patient's history in making clinical decisions relating to quality healthcare decisions.
Analyzes and investigates concerns from patients, caregivers, providers, payers, internal staff, or others.
Handles incoming calls from internal and external customers as well as answering all nurse line and direct calls.
Calls the provider's offices for questions and clarifications. Establishes and maintains effective professional relationships with providers.
Enforces all state, federal, and local laws pertaining to nursing practice as well as Company policies and procedures.
Responsible for coordinating and delivering resources and information to optimize the patient and caregiver experience with specialty therapy with the ultimate goal of improving patient outcomes.
Coordinates the care of high-risk, post-transplantation and other specialty-targeted populations appropriately, efficiently, and cost-effectively.
Assists patients in arranging timely access to services and evaluates social/financial/environmental support adequacy in a culturally sensitive manner.
Utilizes known community resources to link patients with outside services/available programs. May participate in an interdisciplinary care team meeting to problem solve around highly complex care needs.
Interacts with patients via phone and in person to educate, motivate and empower them at every step of their care plan.
Documents patient adherence, side effect, and all information related to treatment and provide patient support as needed. Produce timely, accurate, and quality documentation.
