Regional Home Care Registered Nurse (RN)

FlexStaff is seeking a Home Care Registered Nurse to work for one of our external clients, a non-profit organization that provide and manages long-term healthcare services. In this role you will conduct home visits to provide nursing to patients in home setting. 

We have three openings for the below designated areas (one position for each area):

• Manhattan and Bronx, NY
• Bronx, NY
• Queens, NY

In this role you will be conducting routine visits to perform wound care, administer medications, and complete general health assessments. 

Must have a car and a valid NYS driver’s license. Mileage reimbursement. 

This is a permanent, full-time job. Schedule: Monday – Friday 8:30 AM – 5:30 PM, 37.5 hours per week. Available for weekends/nights coverage as needed.

An ideal candidate would have a nursing degree, a valid NYS RN License and minimum of three (3) years of clinical nursing experience in a certified home health agency (CHHA), acute care, medical surgical, and/or critical care experience.

Responsibilities include, but not limited to: 

• Conduct home visits to provide clinical services, including wound care, medication administration, and health assessments.
• Deliver nursing care based on individualized care plans and physician orders.
• Monitor patient conditions and communicate changes to the care team and physician.
• Collaborate with partner organizations and care coordinators to address patient needs.
• Educate patients and caregivers on condition management, medication adherence, and home safety.
• Document care accurately in the electronic health record system.
• Adhere to clinical protocols, regulatory standards, and infection control guidelines.
• Participate in interdisciplinary case conferences and clinical meetings as needed.
