The HealthBar Nurse Practitioners work directly within the Healthcare Partnership Program (HPP) to advance HealthBar's vision of improving community health and well-being. This on-site role involves providing a range of services, including primary, proactive and preventative care, chronic disease management, health navigation, health optimization and so much more.


Based in Grand Haven, MI, monthly or quarterly out of state travel is required and will be supported.
