The Pharmacy Team within MSHP drives the implementation and ongoing improvement of initiatives that enable Mount Sinai to deliver better value to its patients, its customers (i.e., plan sponsors and payers), its providers, and its partners. Role Summary 

 

Mount Sinai is embracing new payment models that reward value-based arrangements to foster ongoing health improvement. The Pharmacy team is responsible for developing programs, measurements and processes to improve the overall total cost of care, ensuring that we meet critical contract thresholds and that our patients have an effective and seamlessly connected healthcare experience. 

 

The Program Manager for Pharmacy role is both strategic and operational, working directly with clinical and operational leadership to develop strategy, identify and track performance on key indicators and cost indicators, analyze performance against national benchmarks, plan content for physician education initiatives, and serve as the point of contact for the population health team and physician practice managers. The Program Manager will interact with a wide variety of stakeholders throughout the health system and prepare executive presentations about the program’s financial and clinical impact. 

  1. Work directly with the Population Health Pharmacy leadership to support high-impact clinical and operational utilization reduction initiatives. Manages pharmacy program strategy planning, annual operational milestones, program and staff oversight, and key performance indicator tracking.
  2. Serve as operational support for initiatives, contributing to all aspects, including strategy, process design, implementation planning, oversight of staff and ongoing evaluation and improvement 
  3. Structure and perform analyses to measure outcomes, communicate impact, and support strategic decision-making (i.e., financial, clinical, and claims analyses) 
  4. Perform research to inform project team of relevant background content and market trends; synthesizes research into applicable insights that drive effective decision-making
  5. Create and effectively manage program work plans weekly, coordinating across multiple stakeholders to ensure work is accomplished on time 
  6. Proactively and creatively address any challenges that may arise during project execution, communicate risks with leadership timely and proactively
  7. Interface with staff from across the health system (including clinical, contracting, IT, financial, legal, and operations), building collaborative relationships and serving as a face of the population health enterprise 
  8. Develop presentations that communicate the goals, content, and progress of strategic initiatives to leaders across the Population Health enterprise, the broader Mount Sinai Health System, and key external stakeholder groups 
  9. Communicate and manage projects between different departments and teams to support the pharmacy program (Epic/IT, credentialing, finance, etc.)
  10. Support development of financial projections and ROI models for commencement and expansion of a population health/pharmacy program 
  11. Conduct analyses using relevant data and other tools to determine the impact of pharmacy program decisions (e.g., distribution of chronic disease, etc.) and to implement business and/or operational components of the pharmacy program
