The Senior social worker serves as a key point person for cases involving abuse, neglect, guardianship, foster care, adoption, and crisis intervention, often taking a leadership role in clinical situations requiring de-escalation and urgent intervention.Key Responsibilities:
- Collaborate with healthcare teams to facilitate safe, effective, and timely patient care.
- Interpret and communicate complex patient and family needs, modeling caring practices.
- Conduct psychosocial assessments evaluating prior functioning, support systems, reactions to illness, and coping mechanisms.
- Counsel patients and families on emotional, social, and financial impacts of illness or disability.
- Access and mobilize community and family resources to meet patient needs.
- Provide mental health assessments upon physician referral and deliver therapeutic interventions.
- Facilitate complex discharge planning, including end-of-life, guardianship, foster care, and adoption cases.
- Serve as the point person for cases involving child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence.
- Liaise with protective agencies, guardianship, foster care, and mental health providers.
- Support and lead clinical interventions in dangerous or potentially dangerous situations, utilizing de-escalation techniques.
- Utilize effective interviewing techniques with victims, perpetrators, and family members.
- Develop and maintain strong relationships with community agencies, law enforcement, schools, and other stakeholders.
- Education: Master’s Degree in Social Work (MSW) required.
- Licensure: Permanent LSW licensure required; LCSW licensure preferred.
- Experience: 3-5 years preferred in mental health, child and family services, adult protective services, Veteran’s Administration, or substance abuse.
- Certifications: De-escalation certification required within 6 months of hire.
- Skills & Knowledge:
- Knowledge of disease impact and human behavior for psychosocial assessments and crisis intervention.
- Strong understanding of legal mandates related to child and adult protective services.
- Principles of growth and development across the lifespan.
- Expertise in crisis de-escalation and conflict resolution.
- Ability to work effectively with diverse populations, geographically and socio-economically.
- Proficiency in Microsoft Excel and Word.
