Physical Therapist - Outpatient Clinic - Mercy Health Fairfield Medical Specialty/ Neurologic PT Position

Job Description

Mercy outpatient rehabilitation team is a leader in evidenced based neurological care.  All therapist work closely with the talented physicians here at Mercy Health.  The treatment model is impairment based and focused on; a hands on evidence based care that is delivered on a one to one basis.  Therapist are trained in the latest advanced skills.  

 Specifically, you’ll need:

• To evaluate, plan, and implement Physical Therapy services using an impairment base approach according to the standards of practice, governing laws of the State of Ohio and facility and department policies and procedures
• Current licensure as a physical therapist in Ohio
• CPR certification
• Neurological Training and/or Certifications Preferred
• Ability to thrive in a team environment including, Physicians, PT’s and other rehab professionals.
• Ability to educate patients, families, caregivers and patient treatment teams
• Ability to manage a busy case load effectively.

 Job Requirement

• Graduate of accredited program in Physical Therapy. 
• Holds current license from the State of Ohio
• Advanced degree related to a clinical specialty tract, dual education and/or certification in neurological physical therapy, preferred. (Not required)

Job Summary: 

The Physical Therapist completes initial assessments, ongoing assessments and provides skilled therapeutic interventions to patients through the use of their educational knowledge, skill, and ability. This may involve outpatients, inpatients, pediatrics and off-site locations. Services the patients as a part of the entire continuum of professional care. 

 

Essential Functions: 

• Evaluates patients, gathering pertinent information from various sources, and develops an appropriate treatment plan with specific goals. Responsible for ongoing assessment, efficiency of treatment plan and revisions of plan as needed.   

• Provides direct therapy to patients daily following patient’s plan of care and current standards of practice for Physical Therapy . Provides instruction in therapeutic procedures to patients and family.  

• Maintains accurate and concise records on treatment provided and patient's progress. Documents all care provided, coordination of services, orders, goals, etc. in the patient’s electronic medical record in an accurate and timely manner. Provides timely co-signature for supervised physical therapy assistants, if applicable  

• Maintains set level of productivity without jeopardizing quality.  Delivers efficient and effective care to patients meeting and exceeding department standards.
