Job Description Summary

The Direct Access Screening Colonoscopy (DASC) Registered Nurse is responsible for facilitating the coordination and effective operations, ensuring the safe, efficient and timely delivery of patient centered care for patient's undergoing Direct Access Screening Colonoscopy (DASC) or colo-rectal surgery. Patient navigation provides individualized assistance offered to patients, family, and care givers to help overcome health care system barriers and facilitate access to quality medical and psychosocial care. The DASC nurse facilitates timely appointments for screening colonoscopy and/or scheduled follow-up colonoscopy.

They also evaluate patients scheduled for colo-rectal surgery for risk for co-morbidities, then identifies barriers to care and access. They provide information/education prior to the scheduled procedure. The patient perioperative experience starts at the time the patient is scheduled continuing through 30 days post discharge. The goal of the nurse is to facilitate timely access to quality care that meets cultural needs and standards of care for all pre-surgical/ endoscopy patients. The DASC Nurse is within the Ambulatory Surgery Center/ Pre-Admission Testing Department. They promote the quality of patient care and patient/staff satisfaction through effective communication and collaborates between multiple departments and physician partners to ensure seamless care processes. The nurse ensures alignment with the organizational vision strategy of organizing and coordinating care across the care continuum to deliver care at a lower cost while improving health outcomes and optimizing the patient experience. Duties and tasks include: identifying and scheduling appointments, coordinating care among providers through effective communication, ensuring that medical records are available at each scheduled appointment and providing other services to prevent delays.

Major Responsibilities:

• Demonstrates effectiveness in promoting professional practice and resource management that will optimize efficiency and safety through unit operations.
  • Adherence to and holding others accountable to professional guidelines for practice, professional standards, and ethical practice guidelines.
  •  Adherence to and holding others accountable to organizational policy and procedure.
  •  Provides feedback in review and revision of policy, procedure, and clinical practice guidelines/standards.
  •  Promotes interdepartmental collaboration and communication in order to provide seamless, safe, patient centered care.
  •   Develops a partnership and effective communication with the multi-disciplinary care team/ departments to coordinate safe, timely and efficient patient care strategies.
  •   Coordinates patient procedures, particularly for cancer screening related to Digestive Health
• Coordination of Patient Care to Improve Clinical Outcomes
  •  Proactively identifies appropriate patients and utilizes the appropriate screening tools to identify risk factors.
  •   Coordinates with Care management to begin the discharge plan and goals of the Open Access Colonoscopy patient as appropriate.
  •  Oversees the Open Access Colonoscopy patients by functioning as a liaison to the patient care team and physician offices.
  •  Assesses patient needs, communicates the plan, evaluates the effectiveness and revises the plan as needed to reflect changing needs, outcomes, age and risk factors.
  •  Collaborates with PAT to partner with physician offices participating in Open Access Colonoscopy. Maintains effective and up-to-date communication for the interdisciplinary team. Serves as a resource to PAT in relaying pertinent patient care information.
  •  Assesses needs, communicates the plan, evaluates effectiveness, and revises the plan to reflect changing needs, outcomes, age and developmental status. Manages the patient transition through the process.
• Demonstration of Clinical Practice and Professional Growth
  •  Represents Digestive Health Services on specific hospital teams.
  •  Acts as a resource to nursing staff for Open Access Colonoscopy.
