Private Duty LPN – Cleveland, TN

 

Location: Cleveland, TN

Patient: Female

Schedule: Monday through Friday, 10:30 AM – 6:30 PM

Overview:Adoration Home Health is seeking a dedicated and compassionate LPN to provide private duty home care for a female patient in Cleveland, TN. This role involves one-on-one care in the patient's home, ensuring her comfort, safety, and well-being.

 

Key Responsibilities:

• Assist the patient with daily care needs, including physical assistance as required.
• Monitor and document vital signs, intake, and output.
• Provide medication administration as prescribed.
• Collaborate with the healthcare team to maintain high-quality patient care.

Qualifications:

• Active LPN license in Tennessee.
• Ability to lift 70-80 pounds as needed for patient care.
• CPR certification required.

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice• Provide patient and family education according to plan as outlined by the RN• Monitor patient’s vital signs and make specific measurements and observations• Monitor and record intake and output, and report significant changes immediately• Observe, assess and record patient’s physical and mental condition and report any changes• Administer medications as instructed and ordered by the physician and instructed by the RN• Attend and participate in staff meetings and in-service education programs, as assigned

Qualifications

• Active LPN license in good standing• Previous home health, private duty, skilled nursing or hospital experience preferred• Ability to build rapport with patients and family members• Strong problem solving and critical thinking skills• Reliable transportation or valid driver’s license with current automobile insurance
