The Clover Home Care team is dedicated to improving lives by providing compassionate, reliable, and equitable care to seniors in their homes. Our team-based care model is tailored to the personalized needs, values, and preferences of our members, their families, and caregivers. Our interdisciplinary Supportive Care team includes physicians, nurse practitioners, nurses, social workers, medical assistants, behavioral health specialists, and others who collaborate and coordinate to deliver exceptional home-based care.

As an In-Home Care Physician Assistant (PA) you will play a vital role in delivering high-quality medical care to patients in the comfort of their homes. You will conduct comprehensive assessments, develop and manage personalized care plans, and provide medical treatments, including prescribing medications and managing chronic conditions. In addition, you will regularly monitor patient progress, adjusting care plans as needed, and offer valuable education to members and their families on managing health conditions. The PAs collaborate closely with the entire care team to ensure cohesive care delivery that is aligned with our patients’ goals, values and preferences. Through their expertise and direct care, our Physician Assistants enhance healthcare access, improve patient outcomes, and support overall well-being.

As a In-Home Care Physician Assistant, you will:

  • Be accountable for the care and outcomes of a cohort of patients to be managed directly, and in close collaboration with the Physician Team Lead.
  • Visit patients primarily in their homes, as well as through telehealth when appropriate.
  • Contribute medical expertise during Interdisciplinary Team meetings to structure care plans that reflect:
  • Patients' prognoses and disease trajectory, inclusive of goals of care and advanced care planning.
  • Primary drivers of hospitalization/ED risk and interventions to mitigate these.
  • Proactively involve other providers on the patients’ care team, including PCPs, specialists, home health, hospice and home care organizations.
  • Determine the need for consultation from medical, surgical, rehabilitative, and/or psychiatric specialties, which may include deprescribing and simplification where clinically appropriate.
  • Determine the need for, and facilitate care by, an emergency department, hospital, skilled nursing facility, home health agency, hospice and/or community-based organization as appropriate and consistent with patients’ goals of care.
  • Complete documentation of all clinical encounters in a timely and accurate manner in accordance with practice standards, including proprietary Clover tools such as Clover Assistant.
  • Participate in the on call rotation as set by the Medical Director.

Success in this role looks like:

  • How is success measured in the first 90 days? Practice in accordance with the integrated care model to effectively and efficiently care for your patient panel. 
  • How is success measured in the first 6 months? Achieve high performance in clinical and process outcomes as determined by the practice.
  • How will success be measured in the future? In addition to the above, active participation in practice improvement activities in collaboration with the physician lead, practice manager and medical director.

  • You have an active NCCPA Certification.
  • You have an active DEA and CDS registration.
