GENERAL DESCRIPTION:

 

The primary responsibility of this position is to provide Care Coordination for the patient’s pregnancy and postpartum period.  Care coordination services include assistance with obtaining community resources; ensuring prenatal medical and postpartum services are received; providing home visits as part of case management; and referring clients to community agencies/partners. All listed efforts are intended to promote safe and healthy pregnancies, deliveries, and birth outcomes.  This position will also represent the health department at community meetings and on projects involving partnering agencies.

 

 

DUTIES AND RESPONSIBILITIES:

 

The functions listed below are those that represent the majority of the time spent working in this position. Management may assign additional functions related to the type of work of the position as necessary.

 

  • Develop individualized plan of care using the Standardized Plan provided by Community Care of North Carolina.
  • Assist patients in locating a physician for herself and/or for the infant(s).
  • Assist in scheduling/rescheduling appointments.
  • Assist with locating resources for mother and baby.
  • Provide and review educational material on pregnancy, parenting, well-baby care, and birth control.
  • Assume the role as a liaison with the physicians, clinics, and community agencies.
  • Review warning signs and symptoms of preterm labor with the patient at each encounter.
  • Performs family and child developmental assessments as needed.
  • Assist with referrals to community partners for any ongoing
