- Practices sound pharmacoeconomic principles in the treatment selection process by evaluating health care coverage, formulary considerations, and ability to pay when applicable. Documents clinical and financial outcomes.
- Identifies potential adverse drug reactions, drug-drug, drug-disease and drug-food interactions, and incompatibilities affecting patient outcomes.
- Participates on hospital and/or department committees as related to medication safety and/or quality improvement initiatives while demonstrating pharmacy practice leadership.
- Determines appropriateness of medication prescriptions and contacts prescribing practitioner if in doubt of appropriateness of order.
- Processes medication prescriptions to assure timely and accurate dispensing in a form appropriate for the patient.
- Dispenses to and counsels caregivers on the medication. Manages dispensing information through the patient’s profile, reviews patient’s profile before dispensing medication to avoid adverse events.
Education Requirement:
- Bachelor’s Degree from an accredited college of pharmacy, required.
- Post graduate year 1 (PGY-1) or equivalent experience, required.
- Doctor of Pharmacy, preferred.
Licensure Requirement:
Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio, required.
Certifications:
Board certification, preferred.
Skills:
- Clinical competency as evaluated by hiring manager through an independent self-case and/or case presentation in pharmacy.
- Basic skills in using and documenting in an electronic medical record.
- Strong communication and interpersonal skills.
- Aptitude for detail and medication safety skills.
Experience:
Three years of pediatric ambulatory or community care practice experience with responsibility for direct clinical patient care, preferred.
