Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

About the Program

The Adult Outpatient Program (AOP) at Sandra Eskenazi Mental Health Center provides ongoing, community‑based treatment and support for adults living with serious mental illness (SMI) and, in many cases, co‑occurring substance use disorders (SUD). The program emphasizes recovery‑oriented, person‑centered care focused on helping individuals build skills, maintain stability, and remain engaged in treatment over time.

 

Clients served in AOP often require consistent coordination across behavioral health, medical, and community systems. This role supports individuals with complex needs who benefit most from continuity of care, strong therapeutic relationships, and long‑term engagement rather than short‑term or crisis‑only services.

Where You Fit In

The Care Coordinator provides skills training and development to adults receiving outpatient behavioral health services, as required by medical necessity. In collaboration with the interdisciplinary treatment team, the Care Coordinator conducts client assessments and provides skills training and revises person‑centered treatment plans. This role emphasizes consistency, collaboration, and long‑term engagement with clients within a recovery‑oriented model of care.

 

The Care Coordinator advocates for clients to ensure access to needed services, including developing and maintaining strong relationships with community providers and resources that support ongoing recovery and stability.

Am I Qualified?

  • Bachelor’s degree required
    • If incumbent holds a bachelor’s of social work degree, incumbent must also possess a current, unencumbered Licensed Bachelor’s Social Worker license in the State of Indiana
  • Valid Indiana driver’s license and driving record that meets Eskenazi Health’s driving policy requirements

Essential Functions and Responsibilities

•    Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing 
•    Provides skills training and development to clients in group and individual settings as guided by medical necessity outlined in the person-centered treatment plan 
•    In collaboration with the treatment team, the Care Coordinator conducts necessary client assessments as well as revises treatment plans
•    Provides crisis management and emergency interventions including but not limited to cardiopulmonary resuscitation and approved area-specific crisis intervention techniques
•    Reports to treatment team in staffing including but not limited to successes and challenges, assessment of client progress, barriers to success, any level of care changes, etc.
