Major Licensed Practical Nurse (LPN) Responsibilities

• Provide direct patient care to residents and patients of the health and rehabilitation center (skilled nursing home)
• Conduct and document a thorough evaluation of each resident's medical status upon admission and throughout the resident's course of treatment
• Assist in the implementation of individualized treatment plans for each assigned resident
• Oversee Falls Program and Pressure Sore Program
• Supervises staff and oversees patient care in accordance with standards of care and practice and applicable state/federal regulations.
• Follows established facility policies and procedures while providing and coordinating resident care with other facility personnel.
• Supervises Certified Nursing Assistants during shifts and participate in their evaluation
• Other duties as assigned.

Minimum Qualifications for our Licensed Practical Nurse (LPN)

• Current, unencumbered, active nursing license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• You must be qualified, compassionate, and dedicated to a job well done.
