Position Summary

As a member of our Rehabilitation Department, the Physical Therapist plans and implements individualized therapy programs for patients, promoting recovery, mobility, and overall quality of life.

 

Primary Responsibilities

This Physical Therapist is responsible for but not limited to:

• Screen new admissions and evaluate patient needs through assessment and team collaboration
• Develop and modify individualized treatment plans with specific goals and activities
• Provide therapy interventions such as gait training, therapeutic exercises, modalities, and patient education
• Document evaluations, progress notes, and treatment plans accurately and timely
• Collaborate with nursing and interdisciplinary team members to coordinate patient care
• Supervise and mentor therapy assistants and students
• Support discharge planning and care plan development
• Participate in in-service education, departmental quality assurance, and professional growth activities

 

Qualifications

Qualifications

• Bachelor’s degree in Physical Therapy (new grads welcome!)

• Must be licensed as a Physical Therapist (PT) in accordance with the laws of the state

• Minimum of 2 years of experience preferred in a hospital, skilled nursing, or rehabilitation setting
• Strong communication, documentation, and teamwork skills
