About the Opportunity

Pair Team is hiring a full-time Community Health Worker (CHW) to support a high-need Medicaid population through whole-person, relationship-based care. This role is a key part of Pair Team’s Care Management Program, which delivers coordinated medical, behavioral, and social services to individuals with complex health needs.

The CHW will engage members who may be navigating housing instability, chronic health conditions, or behavioral health challenges—helping them stay connected to care, build trust in the system, and meet their personal health goals.

This position primarily allows for remote work; however, it includes regular in-person visits in the community across Clark County (Las Vegas). Typically 1 or 2 days per week will include visiting patients in the community, at their homes and in various sites of care.

 

 

What You’ll Do

• Serve as a primary point of contact for a panel of members, maintaining consistent phone, text, and in-person outreach
• Build long-term, trust-based relationships to support engagement and continuity of care
• Help members navigate healthcare and social service systems, including housing, food access, transportation, and behavioral health
