SOCIAL WORKER | Iredell Home Health - (PRN)

Description

Provides supervision of the Discharge Planner/Social Work Assistant and assists in managing social work services consistent with the agency’s policies and procedures, standards of practice, and regulations as directed by the Executive Director.
Serves as a social work consultant in the home health agency. Provides social work assessments and interventions in accordance to a plan of care and under specific physician orders to patients and families having personal and environmental difficulties in social functioning that predispose the patient to illness and interfere with the patient obtaining the maximum benefit from medical care.

Requirements

• Baccalaureate Degree in Social Work REQUIRED.
• License or certification in Social Work preferred.
• At least one year of hospital social work experience preferred.
• Understanding of principles and practice of social work. Must have working knowledge of community social and health agencies. Must have knowledge of human behavior and personality dynamics.
