Job Description
The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems, and the Bureau of Finance, Administration and Services.
The division's Deputy Commissioner also serves as the Agency's Chief Medical Officer. The Bureau of Brooklyn Neighborhood Health: Formerly known as the Brooklyn Neighborhood Action Centers, the Bureau of Brooklyn, Neighborhood Action Centers, the Bureau of Brooklyn Neighborhood Health will continue to provide oversight and leadership in programming and planning for the Brownsville Neighborhood Health Action Center with satellite programming and planning also occurring at the Bedford and Bushwick District Health Offices. This bureau will support programming, planning and research opportunities to address racial inequities resulting in premature mortality in Central Brooklyn. Reporting to the Bronx Family Wellness Suite Manager and working in partnership with other prenatal health programs out of CHECW and other BNH programs that address the social determinants of perinatal health, the Bronx Community Linkage & Referral Liaison will be primarily responsible for managing and expanding the stakeholder engagement and linkage programming for the Bronx Family Wellness Suite (FWS).
DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:
- With support of the FWS manager and the broader FWS Community Linkage & Referral Liaison team, develop comprehensive portfolio to support stakeholder engagement and linkage referrals.
- Build and maintain relationships with Bronx hospital, clinical, mental health and social service providers which oversee services/programs identified as priority for Bronx FWS clients.
- Lead in the management and facilitation of the Birth Equity Advisory Board, with the support of the rest of the FWS team, to regularly engage the key FWS stakeholders that interface and refer to the programming, including Citywide Doula Initiative partners, doulas, hospitals, clinics and other Bronx community based and social service programs.
- Oversee outreach activities that promote an increased awareness of the Bronx Family Wellness Suite programming including its client referral services into programs like the Citywide Doula Initiative and those providing mental health support services.
- Facilitate referrals into the Citywide Doula Initiative and other perinatal health programs partnered with the Family Wellness Suites for resources that support perinatal health and social service needs.
- Manage all promotion and recruitment into the stakeholder engagement programming for the FWS. This includes the management of electronic registration or presentation platforms as well as the development of flyers or communications requests for these.
- Maintain the records of the stakeholder engagement and linkage referral portfolio of programming, including the management of attendance sheets, assessment tools, and other related forms.
- Support the administrative needs of the stakeholder engagement and linkage referral programming portfolio.
- With support of evaluation colleagues and FWS leadership, develop appropriate continuous quality improvement and/or evaluation tools for all stakeholder engagement and linkage referral programming and participate in their data analysis and regular reporting.
- As a team player, support the promotion of other suite programming portfolios.
- Support the suite with ensuring successful client intake, referrals to services, and management of appropriate client management software.
- As a Certified Lactation Counselor (CLC), or soon to be CLC, provide support in increasing utilization of the FWS lactation room and other lactation clinic programming as necessary.
