Are you passionate about making a difference in the lives of individuals experiencing homelessness in Atlanta? Intown Cares is seeking applicants for its full-time Homeless Services Case Manager position. Intown is a 501(c)(3) nonprofit with a mission to prevent and reverse homelessness and hunger in Intown Atlanta. Founded in 2010, Intown has grown tremendously, more than doubling in size over the past five years. Intown’s homeless services are guided by a Housing First and Harm Reduction philosophy, is one of the largest and most effective teams in the City of Atlanta, and has housed over 1,200 people experiencing
unsheltered homelessness since 2017. The Case Manager will join an experienced, successful, and energetic team while serving with wisdom and compassion.

What You’ll Do
Projects for Assistance in Transition from Homelessness (PATH) is funded through a grant from the Substance Abuse and Mental Health Services Administration (SAMHSA) and the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD). PATH teams conduct street outreach and provide case management and connections to resources for individuals currently experiencing homelessness with behavioral health needs. The PATH program provides interim support and services with the goal of connecting unhoused individuals to long-term resources.

This is an exciting opportunity to join our front-line outreach team and play a vital role in connecting people experiencing homelessness to permanent housing solutions. You'll work with our PATH team to collaboratively identify encampments and neighborhoods where unhoused neighbors are living and who may need connection to services.

The PATH Case Manager will:

  • Build trust with individuals and families experiencing homelessness in your region
  • Conduct assessments to understand client needs and barriers to housing, including substance use and mental health concerns
  • Work collaboratively with our Licensed Clinical Social Worker to address clients; mental health needs and connect them with appropriate support services
  • Develop and implement individualized plans to support clients on their path to housing, which may include referrals for disability applications, employment opportunities, and emergency shelters (as requested by clients)
  • Connect clients with emergency services when needed
  • Provide housing navigation services, including transporting clients and walking alongside them to obtain birth certificates, identification documents, TB tests, income verifications, and mental health services
  • Upon receiving housing referrals, assist clients in connecting with the housing provider for interviews and the lease-up process
  • Collaborate with DBHDD (Department of Behavior Health and Developmental Disabilities) to support unhoused individuals with SPMI (serious and persistent mental illness) in getting connected to services
  • Participate in professional development opportunities to stay up-to-date on best practices.

Qualifications

  • A passion for connecting with people experiencing unsheltered and chronic homelessness
  • Associate or bachelor’s degree in social work, human services, or a related field or equivalent relevant work experience
  • 1-3 years working in homeless services, with at least one year of experience in outreach
  • Experience working in the substance abuse field is a plus
  • Certified Peer Specialists and individuals with lived experiences of homelessness, substance use, and/or mental illness are strongly encouraged to apply
  • Proficiency with Client Track or another HMIS is preferred
  • Valid state drivers' license required

Position Status

The Homeless Services Case Manager is a full-time (40 hours per week), salaried position that reports to a Homeless Services Program Manager.

Work Hours and Location

This position is based in Atlanta. Most hours will be during normal business hours (between 8:00 a.m. – 5:00 p.m.) but will also include some evening and early morning hours. There is some flexibility in start- and end- time and possible administrative work-from-home days.

Travel

This position will require driving, and the Case Manager must have a valid driver’s license and a clean driving record. Intown will reimburse mileage at the federal mileage rate.

Compensation and Benefits

The Case Manager is a salaried position with an annual base salary of $50,000, which will be paid in equal installments every two weeks according to Intown’s regularly scheduled paydays.
