Registered Nurse (RN) – Home Health PRN / Per Diem

What You’ll Do

As a Home Health RN, you’ll visit patients in their homes and help them safely manage their health conditions.

Responsibilities include:

• Completing routine skilled nursing visits

• Performing patient assessments

• Providing medication and disease-process education

• Monitoring wounds, vitals, and overall health status

• Communicating changes in condition to providers

• Coordinating care with therapists and care team members

• Documenting visits in the electronic medical record

• Following HIPAA and patient safety standards

Training & Support

You won’t be sent out alone. We provide:

• Structured onboarding

• Ride-along visits with experienced clinicians

• EMR documentation training

• Ongoing clinical support and case consultation

Requirements

• Active, unrestricted RN license

• At least 1 year of nursing experience in any setting (hospital, SNF, clinic, rehab, etc.)

• Reliable transportation, valid driver’s license, and auto insurance

• Ability to travel within the Lafayette coverage area

• Comfort working independently in the field

• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

Preferred (not required):

• Home Health or Hospice experience

• HomeCare HomeBase (HCHB) experience

• ASN or BSN

Important Information

This is a PRN/per diem position and is not benefit-eligible (no health insurance, PTO, or 401k).
PRN nurses are scheduled based on availability and patient needs.
