Full time, part time, weekday and/or weekend positions available - including morning, schedules.
Key/Essential Functions & Responsibilities
Ensures health, wellness and safety by providing appropriate supervision of the people we support and by managing crisis situations while adhering to policies, procedures and regulations.
Maintains a clean, safe and organized therapeutic environment.
Ensures person-centered, individualized plans are implemented and assists with a variety of tasks for people we support, as appropriate/authorized, including:
Providing personal care and assisting with activities of daily living (ADLs) including intimate care and personal hygiene (e.g. bathing, dressing, and toileting), laundry and housekeeping
Ensuring that medication is administered as prescribed, in accordance with applicable guidelines
Teaching/developing skills
Supporting person-centered advocacy with the individuals
Assisting in communication, self-expression and resolving disputes amongst individuals
Facilitating ethical decision making
Providing supervision for and/or planning and preparing balanced meals and snacks for people we support in accordance with dietary restrictions and guidelines of specific individuals including for modified diets or liquid consistencies.
Transporting and escorting individuals on recreational activities, medical appointments, home visits etc.
Assisting people with making purchases, as well as safeguarding personal allowance funds and any additional funds provided
Under supervision of the Registered Nurse (RN) and/or Licensed Practical Nurse (LPN) with appropriate training, regularly provides additional supports for individuals with complex medical needs as indicated in individuals’ Life, Behavior Support and/or Plans of Nursing Service (PONS) including, but not limited to:
Assisting with transfers, turns, and lifts, to or from bed, wheelchair, prescribed equipment (e.g. Hoyer lifts), or agency vehicle using the appropriate body mechanics and/or safe person handling procedures.
Gathering/stocking medical supplies
Measuring and documenting food and liquid intake and/or urinary and fecal output and reporting changes to medical providers or nursing staff
Checking vital signs (e.g. temperature, blood pressure, heart rate)
Scheduling/following up on appointments in coordination with Health Care Specialist and appropriate nursing staff
Performing catheter care
Assisting with the care of ostomies (e.g. colostomy and ileostomy)
Providing enteral nutrition (e.g. tube feedings)
Taking care of wounds as per medical provider’s orders
Cleans rooms and bed linens and handles/disposes of sharps, bodily waste and/or fluids as per OSHA guidelines and Exposure Control Plan to support and maintain a clean and safe environment.
Collects, records, and summarizes data on individuals’ observable behavior.
Observes and reports any changes in behavior, or other signs and symptoms, to clinical and/or leadership team.
