Recreation Therapist, Clinical Time Transition Program
Work related travel reimbursed at predetermined rates
As a Recreation Therapist for the Clinical Time Transition Program (CTTP) you will work with caregivers to develop therapeutic recreational plans for youth (ages 11-17) transitioning out of a hospital setting to both residential and community settings. You will report to the Director of the Hospital to Home (H2H) Program, and you will work onsite with community travel. You will work in a hospital, residential, and community settings, with occasional travel for client support.
Responsibilities:
- Conduct individualized recreational assessments to identify strengths, interests, and therapeutic needs
- Develop personalized therapeutic recreation plans in collaboration with caregivers and the treatment team
- Establish goals that support emotional regulation, and community integration
- Lead structured recreational activities that promote skill-building and self-confidence
- Incorporate evidence-based interventions such as art therapy, music therapy, outdoor recreation, and team-building exercises
- Adapt activities to meet the needs of each youth, considering cognitive, emotional, and physical abilities
- Educate caregivers on the benefits of therapeutic recreation and provide guidance on implementing activities at home and in the community
- Partner with community organizations to connect youth with recreational resources and programs
- Facilitate group sessions for caregivers on recreational strategies to support youth in transition
- Maintain accurate records of assessments, treatment plans, progress notes, and outcomes
- Maintain records of family support services provided, ensuring confidentiality and compliance with HIPAA regulations
- Contribute to case reviews and interdisciplinary meetings, providing insights on recreational therapy interventions
- Develop recreational therapy programs tailored to youth transitioning out of hospital care
- Advocate for the inclusion of therapeutic recreation as an essential component of transitional care plans
- Stay informed on best practices and new trends in recreational therapy for youth
- Establish activities to meet diverse needs and backgrounds
Requirements and Education:
- Bachelor's or Master's degree in Recreational Therapy, Therapeutic Recreation, or a related field required
- 1+ years of experience working with youth in behavioral health, hospital, residential, or community settings required
- 1+ years of experience with youth with mental health and developmental disability needs and systems required
- Valid NYS Driver’s license required.
- Must pass a physical
