Responsibilities
Patient Assessment & Care Planning
Conduct comprehensive initial and ongoing patient assessments.
Develop, implement, and update individualized care plans based on patient needs and physician orders.
Identify therapeutic, preventive, and rehabilitative interventions to support optimal patient outcomes.
Direct Patient Care
Provide skilled nursing services in accordance with physician orders and the State Nurse Practice Act.
Administer medications, treatments, and clinical interventions as needed.
Monitor patient progress and adjust care plans accordingly.
Educate patients and caregivers on disease management, medications, and health maintenance.
Care Coordination
Serve as the primary coordinator for all aspects of patient care.
Collaborate with physicians, therapists, social workers, home health aides, and community providers to ensure seamless care delivery.
Advocate for patients and help navigate resources that support independence and wellness.
Documentation & Communication
Complete accurate and timely clinical documentation.
Communicate changes in patient condition promptly to physicians and obtain necessary orders.
Ensure compliance with all agency, state, and federal regulations.
Patient & Family Education
Empower patients and families through education and support.
Involve caregivers in the care planning process to promote confidence and successful outcomes.
Supervision & Leadership
Provide direction, supervision, and support to home health aides.
Conduct required supervisory visits and evaluations to ensure high-quality patient care.
Serve as a clinical resource and leader within the care team.

QualificationsQualifications:
Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.
Current RN licensure Β and CPR certification.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Licensed driver with automobile that is in good working order
