Job Description
The Clinical Pharmacist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Clinical Pharmacist coordinates and provides the pharmaceutical care of patients for assigned clinical unit(s).
Oncology experience preferred.
Rotating schedule: 7am-3:30pm, 8a,-4:30pm and 9:30-6pm. Needs to be able to work all 3 schedule times.
Responsibilities:
- Manages drug therapy protocols as a part of an interdisciplinary team.
- Provides drug information and education to medical staff and patients.
- Reviews, interprets, and evaluates medication orders to identify, prevent and resolve potential or actual drug-related problems.
- Recommends formulary alternatives when appropriate.
- Supervises the compounding and dispensing of medications for delivery to patient care areas.
- Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
- Performs other duties as assigned
Qualifications
Required:
- Bachelor of Science in Pharmacy from an accredited College of Pharmacy
- Illinois licensure as a Pharmacist
- BLS certified within 6 months of hire
Preferred:
- 2 years of experience
- Doctor of Pharmacy
- Board Certifications
- ALS certificate
