Job Description Summary:

To competently interpret the patients history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. Based on problems identified, the occupational therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Actively participates in departmental functions.

Responsibilities And Duties:

Driving is an essential function of the job.

90% Direct Patient Care - Evaluates and treats patients after physician referral to:
Evaluate patient’s motor, cognitive, sensory integrative and/or psychosocial function.
Develops and implements plan of care based on evaluation findings and appropriate treatment techniques.
Implements plan of care based on appropriate occupational performance frame of reference(s): bio-mechanical, sensorimotor, and/or rehabilitative.
Attends multidisciplinary patient care meetings. Plans and coordinates discharge plan with multidisciplinary team.
Educates patient and caregiver/family.
Recommends Medical Equipment required for discharge.
Provides information to physicians, nurses, case managers, social workers, and other team members.

Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties.

5% - Indirect Department Functions:
Supervises patient assignments and duties of Certified Occupational Therapy Assistants (COTA) and rehab technicians.
Is involved in department quality improvement through utilization review and quality assurance program.
Takes active role in orienting new staff members.
Serves as clinical instructor for OT and COTA students.
Educates other disciplines and medical/hospital personnel on the topic of OT purpose, goals and treatment policies and procedures.
Represents department at problem-solving and committee meetings.
Assumes other duties delegated by management.
There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.

Minimum Qualifications:

Master's Degree (Required)BLS - Basic Life Support - American Heart Association, DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles, LIC - Licensed in Field - State of Ohio, OT - Occupational Therapist - American Occupational Therapy Assoc.
