Occupational Therapist - Per Diem /Full Time (PACE)

Role Scope:

We are looking for a dynamic, service-oriented occupational therapist who is passionate about supporting elderly persons in their journey. The occupational therapist will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. In addition to clinical services, there is an opportunity to shape the company’s occupational therapy program. 

Participant Care

• Provides clinical occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants.
• Participates in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination in a collaborative spirit.
• Assesses condition, creates a treatment plan and discusses implementation of therapy with participants and caregivers.
• Instructs family members, caregivers, and staff in methods to maximize safety and functional abilities in performing Activities of Daily Living (ADL) with documentation in medical record notes.
• Makes home visits as needed and documents activities including participant/caregiver response in participant’s chart.
• Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of DME.
• Documents therapy services provided and performs department record-keeping in keeping within documentation requirements.

Administration and Program Development

• Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong.
• Continue to raise the bar.  Constructively seek and share feedback, and help us implement changes in order to improve clinical outcomes and experience for participants.
• Exhibit and honor Habitat’s values.
• Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants.
• Performs data collection, prepares reports and maintains any necessary participant statistics.
• Attends all staff meetings and patient care conferences as scheduled.
• Maintains the quality assurance, safety, environmental and infection control programs.
• Serves as subject matter expert for the development of occupational therapy program standardization, evaluation and team education. 
• Serve as subject matter expert to support quality programs and discipline-specific audits.

Required Qualifications:

• Bachelors in Occupational Therapy from Accredited Program meeting AOTA & ACOTE standards
• National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association.
• Registration with the state of California.
• Requires a state issued driver’s license, personal transportation, and auto insurance as required by law.
• A minimum of one year experience working with the frail or elderly.
• Proof of medical clearance for communicable diseases, including a TB test.
• Proof of all immunizations are up to date.

Preferred Qualifications:

• 2+ years as a practicing OT
• Bilingual: Spanish/Mandarin/Cantonese preferred.
• Masters in OT (MOT), Doctorate in OT (OTD)
