Matrix Providers is hiring a Registered Nurse – Disease Management to join our team of talented professionals who provide health care services to our Military Service Members and their families in at Travis Air Force Base, California.
- Employment Status: Full Time
- Compensation: $62.00/hr + $4.57 Health & Welfare amount
- Schedule: The hours of performance and the facility are Monday through Friday 0730-1630. The schedule is maintained by clinic personnel based on demands of the clinic and appointments booked that day.

The Registered Nurse – Disease Management must have and meet the following:
• Degree: Associates Degree of Nursing.
• Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE) or as otherwise identified in the TO.
• Experience: Six years of broad-based clinical nursing experience with three years of progressively increasing managed care responsibilities with a focus in Disease Management is highly recommended. Experience with disease management, clinical practice guidelines, performance improvement, and personnel management is strongly preferred. Full time employment in a nursing field for at least 12 of the last 18 months is mandatory. Possess a thorough clinical knowledge and breadth of experience to be able to contribute to population level assessment and management, identify appropriate use of CPGs, conduct patient self-care education, and track patient outcomes. Extensive knowledge of DM components: outcomes measurement, collaborative practice, CPGs, patient self –management, population identification processes, and process improvement.
• Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the TO.
• Certification: Certification in disease management related area such as: Chronic Care Professional (CCP) by Health Sciences Institute or Certified Health Coach (CHC) by National Society of Health Coaches; or a patient education body such as: Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators or Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board is highly desirable. It is highly recommended candidate to have at least one of these certifications. Basic Life Support for Healthcare Providers, American Heart Association, or American Red Cross Healthcare Provider Course.
Registered Nurse – Disease Management Core Job Duties:
• Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.
• Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
• Perform physical exam and health histories.
• Provide health promotions, counseling, and education.
• Administer medications, wound care, and numerous other personalized interventions.
• Direct and supervise care provide by other healthcare professionals.
• Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
• Recognize adverse signs and symptoms and quickly react in emergency situations.
• Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
• Make referral appointments and arrange specialty care as appropriate.
• Perform nursing services identified in the TO.
• Conduct research in support of improved practice and patient outcomes.
• Must be knowledgeable of clinical and administrative theories, principles, practices, and procedures underlying nursing practice; a wide range of medical disorders/conditions and disease processes across the lifespan to include pediatrics, adolescence, adults, obstetrics, and geriatrics; assessing patients, including the ability to assess/evaluate telephonically; preventive health schedules, procedures, processes and education methods; disease management and population health principles to
