Job Description Summary
Investigates and assesses allegations of mistreatment and self-neglect of at-risk adults. Offers protective services to prevent, reduce or eliminate the current or potential risk using community-based services and resources in addition to family and friends. Assesses for safety and risk and determines level of intervention required by the Department of Human Services. Assists in recommendation and coordination of services to prevent or alleviate the abuse, neglect, self-neglect, or exploitation.--

Casework (75%):
- Receives assigned reports of at-risk adult abuse, neglect, self-neglect, or exploitation from assigning supervisor.
- Verifies information on referral (address, household composition, history, criminal history) and research for additional information using multiple databases.
- Contacts Reporting Party to gain additional information and insight to determine type of investigation.
- Makes in person contact with the alleged victim in the home, long term care facility, medical facility, or other location within required timeframe of referral.
- Interviews and observes using investigative questioning techniques gathering enough information to verify concerns and determines level of risk.
- Interviews collateral contacts as appropriate.
- Determines if at-risk adult needs protective services.
- Performs client assessments within 45 days of the receipt of the referral to determine level of capacity and needs of client.
- Assessments identify the client's physical status, environmental status, resources and financial status, mental and behavioral status, medical status and support systems.
- Based upon the findings of the investigation and assessment and with input from the client, develops a case plan that includes services or support that maintains a healthful environment and self-sufficiency in the least restrictive setting.
- Plans may include assisting client in applying for social security, Medicare, and/or Medicaid benefits, helping clients locate and set up appropriate medical and/or psychiatric treatment and coordinating home nursing care or other available home care services that will allow the client to remain in their home as long as possible.
- Provides ongoing case management and support to clients as long as the case remains open.
- Visits clients in person according to required guidelines and continues to provide referrals to additional services and resources that become available.
- Works with clients to promote personal skill and self-reliance.
Collaboration and Community Engagement (20%):
- Participates in the Adult Protection team to maintain community awareness of services and to enhance community support for at-risk adults.
- Provides training on adult protection reporting and issues to community members and groups.
- Provides case-specific documentation including development of plans, case notes and contact documentation, service referrals, and other related tasks within regulated timeframes in the State data base.
- Complies with all rules, regulations and procedures pertaining to Adult Protection.
- Participates in scheduled supervision, team meetings, unit meetings and division meetings.
- Performs as an active team member in RED team, focus groups and other professional development work groups.
- Obtains training or additional professional knowledge when needed.
