Make a difference in the community as an Emergency Department Community Health Worker (CHW). With guidance and direction from the Community Health Worker Supervisor and the Senior Director for Care Coordination and Clinical Social Work, this role will work with patients at our UCLA Health Emergency Departments (ED). The ED CHW program engages emergency department patients who have significant unmet social needs, are experiencing homelessness and/or precarious housing, are at risk for STDs, and have substance use disorders. The CHW will provide individualized support, motivational interviewing, and navigation to medical services, housing and social services, addiction treatment, harm reduction, and other community services to address health-related social needs. The goal of the CHW is to provide navigational, case management, and peer support services to patients who present to the ED for a substance use-related visit.
CHW duties include:
- Working collaboratively on an interdisciplinary team utilizing an integrated client care model in conjunction with best practices for Community Health Workers.
- Engaging in effective motivational interviewing, peer outreach, case management, and navigational services to Emergency Department patients experiencing homelessness and/or precarious housing, are at risk for STDs, and substance use disorders.
- Coordinating collaboration with the primary care (PCP), community services, case managers, and social workers, to deliver interventions that will address social needs and maximize patient health outcomes.
