Description

Position Overview

The Visit Nurse provides high-quality, patient-centered, direct nursing care to individuals facing life-limiting illnesses. This role involves making scheduled and unscheduled visits to patients in their homes or residential facilities, ensuring comfort, dignity, and support for both patients and their families.

 

Essential Duties and Responsibilities

  • Conduct in-home visits to assess patient condition, manage symptoms, and provide emotional support.
  • Administer medications and treatments as prescribed, including pain and symptom management.
  • Collaborate with interdisciplinary team members including case manager RNs, physicians, social workers, chaplains, and aides.
  • Educate patients and families on disease progression, care plans, and available resources.
  • Document all care provided and communicate updates to the hospice team.
  • Respond to urgent patient needs and provide crisis intervention when necessary.
  • Ensure compliance with hospice policies, procedures, and regulatory standards.

Qualifications

Required Education/Credentials:

  • Graduate of an accredited nursing program.
  • Current California Registered Nurse license.

 

Required Experience:  Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program.

 

Required Skills and Abilities:

  • Strong clinical assessment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to manage a caseload effectively and efficiently.
  • Compassionate and patient-centered approach to care.
  • Proficiency in electronic medical records (EMR) systems.
  • Valid driver's license and reliable transportation.

 

Schedule Requirements

  • Must be able to work four (4) weekend shifts (four days) per month.
  • Must sign-up to work two (2) holidays per year.

 

Preferred Qualifications

  1. Experience in hospice or home health.
  2. Experience working effectively with an interdisciplinary team.
  3. CHPN certification.
