Sangre de Cristo Community Care is looking for an experienced Registered Nurse to join our homecare team in Canon City, Colorado. Sangre de Cristo Community Care is the Southern Colorado leader in enhancing the quality of life and maintaining the integrity of individuals and their families in need of supportive or end-of-life services by offering symptom management, support, comfort, and compassion.

Responsibilities:

· The RN is the pivotal person in identifying the physical, psychological, social, and spiritual needs of the patient and family.

· Initiates appropriate intervention and support for the patient and family upon admission.

· Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.

This is a Part-Time position: Wednesday-Friday, 8 AM - 5 PM.

Requirements

Qualifications:

· A valid non-restricted nursing license in the state of Colorado is required.

· Must have a valid non-restricted driver’s license.

· Flexibility and availability to work a weekend schedule if needed.

· At least 1 year of hospice or home health experience preferred.
