Duties:

• Completes an initial assessment of client and family to determine home care needs, making necessary revisions as status and needs change.
• Performs assigned nursing procedures, including administration of medications in the care of the client in accordance with the client care plan, nurse delegation, the State’s Nurse Practice Act and physician orders.
•  Monitor patients' progress and adjust treatment plans accordingly.
• Builds personal rapport and establishes trust with clients, family members, and caregivers.
• Effectively communicates and interacts with clients, caregivers and families in a supportive manner
• Provides “best in class” care management to clients

Weekly Schedule:  Monday to Friday Weekends as needed
